This position is responsible for successful implementation of Case Management services in the Healthy Families/Healthy Children program. Recruitment and outreach to referral sources and participants is required. The focus is providing the necessary case management for participants to be successful in attaining their service goals, including but not limited to financial self-sustainability and overall family stability. This full-time position requires a Bachelor’s degree, preferably in a behavioral science field, and two years of experience in social services. A good driving record and reliable vehicle required. Must be bilingual English/Spanish.
